Position Summary

This employee is responsible for direct provision of investigation, assessment, care planning and reassessment of persons over 60 years of age in need of protective services through the agency. Work is reviewed by the Protective Services Care Management Supervisor II through conferences, evaluation, and review of case records.  Work is performed in accordance with established regulations, policies and procedures.  This employee is expected to exercise initiative and independent judgment in discharging their duties and seeking advice and help with the more unusual and difficult cases.

Essential Duties and Responsibilities

 

  • Investigate reports of need for protective service
  • Assess the needs of persons requiring protective service, prepare care plans, authorize service, and reassess the need for continuation of protective service on an on-going basis.
  • Work closely with agency solicitor to prepare involuntary intervention and guardianship petitions; provide testimony at such hearings.
  • Work in close cooperation with Area Agency on Aging sub-contractors, other social agencies, hospitals, clinics, courts, and other community resources in planning to meet needs of clients.
  • Prepare case records, progress notes, write letters, make reports, and maintain records.
  • Participate in inter-disciplinary conferences with other agency staff persons.
  • Provide information and training to professionals and lay persons regarding protective services, Act 28 and Act 13.
  • Complete a standardized comprehensive medical-social assessment of individual care needs to persons requiring intervention.
  • Obtain information from consumers, family members, medical professionals and others as needed in order to identify social, economic, emotional, health and physical problems and plan appropriate resolutions.
  • Maintain a working knowledge of medical terminology, therapies and medications.
  • Educate consumers, support systems and providers regarding available long term care services; encouraging consumer choice and utilization of least restrictive care options.
  • Maintain a working knowledge of community resources to be able to include available resources as a factor in assessing the consumer's circumstances and recommending a comprehensive plan of care.
  • Enter data as required into computer on a timely basis.
  • Participate in relevant training and meetings.
  • Perform intake duties on a rotating basis.
  • Perform other duties as assigned.

 

Essential Requirements

 

  • A Bachelor's Degree which includes or is supplemented by 12 semester hours credit in sociology, social welfare, psychology, gerontology, or other related social sciences and one year as an Aging Care Manager 2 or Aging Program Assessor or three years of experience in public or private social work, including one year of direct aging care management experience and; or two years of experience in public or private social work including one year of direct aging program experience and a bachelor's degree with a social welfare major; or any equivalent combination of experience and training including successful completion of 12 semester hours of college level courses in sociology, social welfare, psychology, gerontology, or other related social sciences.
  • Knowledge of the laws, rules and regulations governing the protective services program.
  • Knowledge of client conditions that permit or require the intervention of court supervision.
  • Knowledge of social casework principles and methods.
  • Knowledge of the availability of community support services for the benefit of elderly clients.
  • Knowledge of current social, economic and health problems and resources as they relate to older adults.
  • Knowledge of individual and group behavior and ways of working effectively with older adults who have social, economic, emotional or health problems.
  • Skill in the development of plans to address the needs of vulnerable older adults.
  • Ability to establish and maintain effective working relationships with clients, other members of the staff, outside agencies, institutions and the general public.
  • Ability to plan and organize work, prepare adequate records and reports, set priorities and maintain a caseload of clients in an effective manner.
  • Ability to clearly express ideas orally and in writing and to interpret laws and regulations.
  • Ability to maintain confidentiality in high pressure situations.
  • Ability to understand and accept the needs and rights of other persons and to work with older adults who are physically disabled or economically disadvantaged.
  • Ability to work effectively with people and aid them to grow in the constructive use of their potential in adjusting to their specific problems.
