DUTIES AND RESPONSIBILITIES:

The Assisted Living Assistant Program Coordinator is responsible for overseeing the day-to-day operations of the CHIPP Assisted Living homes which provide residential services for three mental health consumers per home, who have special physical and/or psychiatric needs requiring one-on-one staff assistance.

The Assisted Living Assistant Program Coordinator is responsible for supervising the CHIPP Assisted Living staff and for assisting the residents served by this program. The Assistant Program Coordinator hours are flexible with day, evening and weekend hours.

The Mental Health Department provides recovery-oriented services, such as crisis plans and Advanced Directives. All service delivery is based on the department’s guiding principles which include a focus on consumer strengths and choice.

 

 

CRITICAL ELEMENTS:

1)        Staff Supervision: Responsible for participating in the interviewing process of prospective staff and to supervise the staff necessary to operate the CHIPP Assisted Living Program. Supervises the Assisted Living Counselors and monitors staff time sheets and expense records, completes personnel evaluations for all direct subordinates and provides direct coverage in emergencies or when unable to staff otherwise.  Assistant Program Coordinator is responsible for ensuring houses are staffed appropriately on the evenings and weekends and securing shift coverage during staff shortages (if there is a vacant shift open at a house).

2)        Direct Services: Ensure that residents of the CHIPP Assisted Living program receive an appropriate quality and quantity of direct care services from the staff. Provides direct care service as needed. Supervises the medication monitoring system.

3)        Facility Operations: Supervise the day-to-day operations of the CHIPP Assisted Living Homes; report maintenance problems and/or the need for repairs; oversee and assist the staff and residents with shopping; house cleaning; and all duties necessary to the successful running of a household

4)        Administrative Services: The Assisted Living Assistant Program Coordinator participates in the Agency administrative process by participating in team management meetings, facilitating quarterly ALC meetings, committee work and coordination with other Agency management personnel. Participates in special activities, programs, etc. as assigned by the Program Coordinator and/or the Director of Mental Health Services.

5)        Will assume the duties of the Program Coordinator in their absence as assigned by the Program Coordinator and/or the Director of Mental Health Services.

ADDITIONAL ELEMENTS:

1)        Rotate on-call time as assigned.

2)        Transport consumers as needed.

3)        Participate in Agency staff meetings, program staff meetings, team meetings and special activities.

4)        Attend a minimum of 24 (twenty-four) hours of training per fiscal year.

5)        Follow and support Agency philosophy, policies and procedures.

6)        Other duties as assigned by the Program Coordinator or Director of Mental Health Services.

 

 

General Requirements & Skills

Minimum Qualifications: Bachelor’s Degree in Social Science or 4 (four) years Human Service experience; 21 (twenty-one) years old; must have had license for 2 years, valid Pennsylvania Driver’s License and reliable transportation. Pennsylvania State Background Check. E.O.E
