Description

DUTIES AND RESPONSIBILITIES:

The Mental Health Department provides recovery-oriented services, such as WRAP, Personal Medicine, Power Statements and Advance Directives. All service delivery is based on the department’s guiding principles which include a focus on consumer strengths and choice.

The Assisted Living Counselor is responsible for providing direct care service to residents in the CHIPP Assisted Living Program. These residents have special physical and/or psychiatric needs requiring one-on-one staff assistance daily.

Full-time Assisted Living Counselors will be scheduled to work a rotating shift consisting of three (3) working days on schedule and six (6) days off schedule.

CRITICAL ELEMENTS:

1)        Medication Monitoring: Implement the medication monitoring system as instructed (monitor medications taken, complete medication logs, etc.)

2)        Direct Care: Respond to any resident emergency during the shift and follow the program situation management directives. Provide supportive counseling when needed. Provide residents assistance with activities related to daily living skills i.e. cleaning, laundry, socialization, hygiene, etc. and as needed in an amount appropriate to the needs of the individual residents.

3)        Documentation: Accurately complete required documentation i.e. Electronic Progress Notes, Incident Reports, Communication Logs, reporting PRN’s, etc.

4)        Household Duties: Assist the assisted living home staff and residents with cleaning and all duties necessary to the successful running of a household.

5)        Resident Safety: The Assisted Living Counselor has frequent direct contact with consumers and should be observant for safety issues and concerns and ready to respond promptly; correcting what they can and reporting what they cannot to the Assistant Program Coordinator or on-call staff.

 

 

ADDITIONAL ELEMENTS:

1)        Participate in Agency staff meetings, program staff meetings, trainings and special activities.

2)        Contact supervisor for any emergency.

3)        Transport CHIPP residents as needed.

4)        Serve actively as team member of the CHIPP Program i.e. assist co-workers in providing coverage during their absences; assist in providing training for Assisted Living counselors and assist with program projects.

5)        Participate in quality improvement and resident education efforts.

6)        Attend a minimum of 24 (twenty-four) hours of training per fiscal year.

7)        Follow and support Agency philosophy, policies and procedures.

8)        Assume other duties as assigned by the Program Coordinator or the Assistant Program Coordinator.

General Requirements & Skills

Minimum Qualifications: One (1) year college credits in Social Science or one (1) year Human Service experience; 21 (twenty-one) years old; must have had license for 2 years, valid Pennsylvania Driver’s License and reliable transportation. Pennsylvania State Police Background Check.
