General Responsibilities

Responsible for performing a variety of duties to provide quality nursing care to residents; performing various documentation and communication duties; ensuring safety and care of resident environment; providing residents with dignity and respect; achieving goals as established in the department’s annual operating plan; coordinating work within the department, as well as with other departments; supervising assigned staff; reporting pertinent information to the immediate supervisor; responding to inquiries or requests for information.

Basic Qualifications  

Education/Training: Must possess a current unencumbered LPN/ RN license in the Commonwealth of Pennsylvania.  We will consider a graduate nurse with the stipulation that an RN license will be achieved within 90 days from date of hire.
Skill(s): Speak and understand English; proficient reading, writing, grammar, and mathematics skills; proficient interpersonal relations and communicative skills; proficient decision-making skills; proficient assessment skill; thorough knowledge of current Department of Health regulations; current CPR certification; demonstrated supervisory skills. Refer to separate analysis for physical requirements of the essential functions with or without accommodation.

Experience: A minimum of one (1) years’ experience as an LPN/ RN preferred.

Essential Duties

  1.  Interacts with residents, clients, staff, and the general public in ways that demonstrate the agency’s commitment to courtesy, service, and hospitality befitting a church related organization
  2. Performs a variety of duties to provide quality nursing care to residents as follows
  3. Admits, transfers, and discharges residents in accordance with Agency standards and physician orders
  4. Provides emergency response/care to residents in the skilled care center and within the retirement community.
  5. Prepares and administers medications and treatments as ordered by the physician using current, accepted standards of nursing practice and quality assurance.
  6. Ensures that Schedule I-IV Narcotic Records are accurate for shift; reports any discrepancies to the supervisor.
  7. Understands and utilizes emergency medication dispensing system.
  8. Administers skilled services; assists with physician visits, physical examinations, and chart reviews; provides direct nursing care as needed.
  9. Informs resident’s attending physician and family when there is a change in the resident’s condition or other pertinent information regarding the resident’s status
  10. Performs assessments of resident condition and communicates findings with physician.
  11. Performs IV therapy per standards.
  12. Performs various documentation and communication duties as follows:
  13. Maintains accurate and comprehensive resident information; charts progress notes on all residents in a descriptive manner
  14. Transcribes and verifies physicians’ orders to appropriated areas of the medical record
  15. Completes 24-hour report recording all changes, incidents, and new orders, noting follow-up for 24 hours; communicates this information to the oncoming shift.
  16. Assists in preparing medication/treatment records for upcoming month.
  17. Communicates with interdepartmental staff and ancillary services when coordinating nursing services to assure that new move ins, move outs, and transferred residents information and orders are completed; assures that the residents total regimen of care is maintaine
  18. Ensures safety and care of resident environment as follows
  19. Assures that all work areas are kept clean, free of clutter, and are adequately stocked
  20. Assures that all aspects of resident care are done properly and timely and resident areas are free of obstacles to prevent accidents and injuries.Performs all tasks in a safe manner, following established safety and infection control practices and procedures, including standard precautions and proper hand washing technique.
  21. Communicates with interdepartmental staff and ancillary services when coordinating nursing services to assure that new move ins, move outs, and transferred residents information and orders are completed; assures that the residents total regimen of care is maintained.
  22. Follows limited lift standard
  23. Reports all incidents/accidents to self, resident, or others immediately.
  24. Provides residents with dignity and respect as follows:
  25. Identifies self and purpose when entering resident’s room; keeps doors and curtains closed when providing direct care for residents; ensures proper fitting and clean clothing is worn by residents and grooming is successfully accomplished; addresses residents by their full names, unless otherwise specified.
  26. Prevents, identifies, investigates, and reports suspected abuse, neglect, exploitation, and improper use of chemical and physical restraints, in accordance with Agency standards
  27. Knows and respects all resident rights, while maintaining the confidentiality of all resident care information.
  28. Oversees team members in the timekeeping process, ensuring accurate submission of data:
  29. Follows up with team members under their supervision to ensure timekeeping process is complete and submitted accurately with no errors.
  30. Educates team members regarding requirements for timekeeping, missed punches, consequences, etc.
  31. Enters department’s timekeeping data accurately and submits data within practice guidelines.
  32. Evaluates data related to resources and program and makes appropriate corrective actions with team members, if necessary, to ensure compliance with all rules, regulations, and agency standards.Coordinates specific work tasks with other staff within the department as well as with other departments in order to ensure the smooth and efficient flow of information.
  33. Interacts with all colleagues in ways that assist one another in the successful performance of their duties.
  34. Provides support, encouragement, and direction to the individuals under his/her direct supervision; acts as a resource and a role model for staff in problem solving and properly communicates within the established chain of command.
  35. Creates an atmosphere that promotes a positive and respectful work environment.
  36. Abides by the current laws and organizational standards and procedures designed and implemented to promote an environment which is free of sexual harassment and other forms of illegal discriminatory behavior in the work place
  37. Cooperates with, participates in, and supports the adherence to all Agency standards, procedures, and practices in support of risk management and overall safety and soundness and compliance with all regulatory requirements, i.e., HIPAA, OSHA, etc.; insures that the department and all staff adhere to the same.
  38. Abides by and supports the organization’s code of conduct by exhibiting behavior that is set forth in the code of conduct.  Performs duties in accordance with the code of conduct and completes annual compliance training.
  39. Directly supervises assigned staff as follows:
  40. Orients, and makes provisions for the proper training of staff; regularly reviews the competence and performance of this group.
  41. Makes certain that all relevant information is properly communicated to the group in order to give them every opportunity to perform their duties properly.
  42. Organizes, schedules, and distributes work among assigned staff
  43. Conducts regular meetings with staff.
  44. Works closely with the group in implementing an effective human resource management program to insure a competent, motivated group of team members.
  45. Administers Human Resources Standards of Practice and other procedures as established by agency standards.
  46. Recognizes opportunities for staff growth development.
  47. Assumes duties of Resident Care Supervisor as assigned
  48. Reports pertinent information to the immediate supervisor as requested, or according to an established schedule; compiles information as necessary or as directed and provides data to appropriate staff
  49. Responds to inquiries or requests from customers, other staff, etc., within given timeframes and within established policy.
  50. Performs nursing duties specific to wound care as assigned.
