Basic Qualifications

Education/Training:Graduate of an accredited school of nursing; possess a current unencumbered LPN license in the Commonwealth of Pennsylvania; current CPR/AED certification (or obtaining CPR/AED certification within 30 days of starting the position).

Skill(s):Proficient reading, writing, grammar, and mathematics skills; proficient interpersonal relations and communication skills; proficient decision-making skills; proficient evaluation skill; thorough knowledge of current Department of Public Welfare regulations for personal care home.

Experience: None required.

General Responsibilities

Responsible for performing a variety of duties to provide quality nursing care to residents and to coordinate total personal care for residents; performing various documentation and communication duties; ensuring safety and care of resident environment; providing residents with dignity and respect; coordinating work within the department, as well as with other departments; supervising assigned personnel; reporting pertinent information to the immediate supervisor; responding to inquiries or requests for information.

Essential Duties

  1. Supports the mission and values of SpiriTrust Lutheran which is: SpiriTrust Lutheran, witnessing to the Gospel of Jesus Christ, provides exceptional health, human, and senior living services by supporting persons in achieving an optimal quality of life.
  2. Interacts with residents, clients, staff, and the general public in ways that demonstrate the agency’s commitment to courtesy, service, and hospitality befitting a church related organization.
  3. Performs a variety of duties to provide quality nursing care to residentsand to coordinate total personal care for residents as follows
  4. Assists with the administration of medication and treatments.
  5. Maintains accurate narcotic records and destroys narcotics as required; reports any narcotic discrepancies to supervisor.
  6. Performs routine administrative tasks, such as completing lab slips, scheduling appointments and transportation, preparing podiatry lists and maintenance requests.
  7. Admits, transfers, and discharges residents following physician’s orders and in accordance with Agency policy.
  8. Monitors all lab work to assure that all testing is done in compliance with physician’s orders and state and federal regulations; consults with the physician regarding abnormal results; follows through with suggested and approved actions.
  9. Notifies physician of any changes in condition based on the evaluation.
  10. Administers professional services such as Foley care, dressing applications and changes, hot and cold packs, oxygen therapy, etc. using appropriate aseptic technique.
  11. Promotes independence and maximum function in all residents.
  12. Performs various documentation and communication duties as follows:
  13. Maintains accurate and comprehensive resident information; documents progress notes on all residents.
  14. Transcribes physicians’ orders according to departmental guidelines.
  15. Completes 24-hour report recording all changes, incidents, and new orders, noting follow-up for 24 hours; communicates this information to the incoming shift.
  16. Participates in the initiation and update of Support Plan for residents.
  17. Contributes to discussions about approaches and interventions.
  18. Communicates with interdepartmental staff when coordinating health services to assure that new admissions, discharges, and transferred residents information and orders are completed; assures that the residents total regimen of care is maintained.
  19. Documents resident and team member incidents/accidents.
  20. Maintains open and positive communication with families or responsible parties, informing them of significant changes, support plan decisions, or other pertinent information regarding resident status.
  21. Ensures safety and care of resident environment as follows:
  22. Assures that all work areas, including resident rooms, dining areas, tubrooms, nurse’s station, and laundry rooms, are kept clean, free of clutter, and are adequately stocked.
  23. Assures that bed making and laundry are done properly and timely, residents’ personal supplies are in appropriate places, and areas are free of obstacles to prevent accidents and injuries.
  24. Performs all tasks in a safe manner, following established safety and infection control practices and procedures, including universal precautions and proper hand washing technique.
  25. Attends in-service training to understand and participate in drills in order to demonstrate knowledge of assigned responsibilities with disaster, fire emergency, and elopement procedure; performs assigned responsibility in an emergency
  26. Provides residents with dignity and respect as follows:
  27. Identifies self and purpose when entering resident’s room; addresses residents by their full names, unless otherwise specified.
  28. Keeps doors and curtains closed when providing direct care for resident.
  29. Prevents, identifies, and reports actual or suspected abuse, neglect, exploitation, and improper use of chemical and physical restraints, in accordance with Agency standards.
  30. Knows and respects all resident rights, while maintaining the confidentiality of all resident care information.
  31. Assists residents with properly storing and retrieving their belongings.
  32. Oversees team members in their timekeeping process, ensuring accurate submission of data:
  33. Follows up with team members under their supervision to ensure timekeeping process is completed and submitted accurately with no errors.
  34. Educates team members regarding requirements for timekeeping, missed punches, consequences, etc.
  35. Enters department’s timekeeping data accurately and submits data within practice guidelines.
  36. Evaluates data related to resources and program and makes appropriate corrective actions with team members, if necessary, to ensure compliance with all rules, regulations, and agency standards.
  37. Interacts with all colleagues in ways that assist one another in the successful performance of their duties.
  38. Provides support, encouragement, and direction to the individuals under his/her direct supervision; acts as a resource for staff in problem solving and properly communicates within the established chain of command.
  39. Creates an atmosphere that promotes a positive and respectful work environment.
  40. Coordinates specific work tasks with other personnel within the department as well as with other departments in order to ensure the smooth and efficient flow of information.
  41. Abides by the current laws and organizational standards and procedures designed and implemented to promote an environment which is free of sexual harassment and other forms of illegal discriminatory behavior in the work place.
  42. Cooperates with, participates in, and supports the adherence to all Agency standards, procedures, and practices in support of risk management and overall safety and soundness and compliance with all regulatory requirements, i.e., HIPAA, OSHA, etc.; insures that the department and all personnel adhere to the same.
  43. Abides by and supports the organization’s code of conduct by exhibiting behavior that is set forth in the code of conduct.  Performs duties in accordance with the code of conduct and completes annual compliance training.
  44. Directly supervises assigned personnel as follows:
  45. Recruits, selects, orients, and makes provisions for the proper training of staff; regularly reviews the competence and performance of this group.
  46. Makes certain that all relevant information is properly communicated to the group in order to give them every opportunity to perform their duties properly.
  47. Organizes, schedules, and distributes work among assigned personnel.
  48. Conducts regular meetings with the staff.
  49. Works closely with the group in implementing an effective human resource management program to insure a competent, motivated group of team members.
  50. Administers HR Standards of Practice and other procedures as established by agency standards.
  51. Adheres to all listed competencies for the position.
  52. Reports pertinent information to the immediate supervisor as requested, or according to an established schedule; compiles information as necessary or as directed and provides data to appropriate personnel.
  53. Responds to inquiries or requests from customers, other personnel, etc., within given timeframes and within established policy.
  54. Completes educational requirements as mandated by state and federal agencies and to maintain competencies
