SpiriTrust Lutheran is now hiring a FT Day LPN Resident Care Coordinator

Your Experience Includes

  1. Graduate of an accredited school of nursing with a current unencumbered LPN license
  2. Current CPR/AED certification (or obtaining certification within 30 days of hire)
  3. A minimum of one year experience in a related position normally required
  4. Knowledge of State and Federal regulations, including Assisted Living regulations
  5. Proficient assessment and organizational skills
  6. Demonstrated communication and decision-making skills

Clinical Oversight and Administration

  • Oversees internal and external health related services and administers appropriate health care of residents in accordance with the Assessment and Support Plan
  • Facilitates communication and documentation related to resident health care
  • Supports, cooperates with and participates in the adherence to all SpiriTrust Lutheran standards, procedures and practices in support of risk management, compliance with regulatory requirements, and overall safety

Management Responsibilities

  • Oversees and manages Resident Care Partners, supporting the development and maintenance of a strong health services program
  • Provides guidance, support and encouragement to the care team with a focus on vision and purpose
  • Establishes an environment that promotes dignity and respect for residents and team members
  • Facilitates communication among the care team, supporting information sharing and problem-solving
