Description
- $1,000.00 Sign-on Bonus for applicants hired between 5/19/22 and 2/28/2023. Paid upon completion of 90 days of employment.
- Full-time position, salaried exempt
- $930/week to start; an additional $30/week upon successful completion of the 90-day introductory period
- Direct support is NOT required in this role

Responsibilities include:
- Coordination, completion, and development of assessments, plans, and protocols
- Participation in the development and implementation of the Individual Plan (IP), IP annual update and IP revisions
- Coordinating, supervising, monitoring and evaluating services provided to the individual
- Coordinating and conducting the training of direct support workers in the content of health and safety needs relevant to each individual
- Navigating a digital platform
- Functioning well in a team dynamic, including cross-departmental teams
- Proficiency with inputting, measuring, analyzing, and interpreting an Electronic Health Record
General Requirements & Skills
- Must have one of the following, per regulations:
- Master’s degree and 1 year of experience working directly with individuals with an intellectual disability or autism
- Bachelor’s degree and 2 years of experience working directly with individuals with an intellectual disability or autism
- Must possess a valid driver’s license and at least 3 years of good driving experience with your license, and must meet our driver eligibility criteria
